What do I Need to Verify My Eligibility?

The documentation you will need to verify your employment status will vary based on your occupation. Use the guide below to make sure you have everything you need. Then, from the Ford First Responder Recognition Home Page, log into ID.me and upload your documentation.

  • Law Enforcement Officers: valid law enforcement ID card, paystub, or Standard Form 50 (SF-50).
  • Firefighters: must have an active Proboard certification and a U.S. Social Security Number. They can then verify their status using one of the following: a valid firefighter ID card, recent training completion certificate, a copy of a valid and professionally-issued certification illustrating firefighter status, paystub.
  • EMTs and Paramedics: Copy or picture of their valid EMT/Paramedic ID card, recent training completion certificate, a copy of a valid and government-issued certification illustrating EMT/Paramedic status, paystub.
  • 911 Dispatcher: Recent training completion certificate, a copy of a valid and professionally-issued certification illustrating 911 Dispatcher status, paystub.
